Lasers used for skin rejuvenation

Lasers currently being used for skin rejuvenation can be broadly classified into two groups. These are ablative lasers that burn off layers of skin and non-ablative lasers that just heat the tissues to stimulate synthesis of new collagen.
Non-ablative lasers require multiple sessions and results are not as striking as those with the use of ablative lasers which in general are considered more effective.

 

Ablative lasers are used to burn off top layers of the skin in a controlled manner causing controlled tissue injury. Resultant healing response then produces new younger looking plumper skin.

History of laser resurfacing

Lumenis introduced ultrapulse CO2 laser resurfacing in 1990s. This has been considered king of ablative lasers and “Gold Standard” for skin rejuvenation and removal of wrinkles, pigment changes, sun damage, acne scars and skin resurfacing.
Although conventional CO2 laser resurfacing achieved great results most people did not want to have the procedure repeated because of a prolonged and difficult recovery period. Social downtime could last three weeks, prolonged facial redness could last three months and in addition there was risk of bacterial, viral or yeast infections with possible scarring and also delayed risk of pigment changes which could be darkening or lightening of skin color. To avoid the risks of complications and reduce recovery time recently new technology called fractional laser resurfacing has been developed. Here the laser beam is broken up or fractionated into many small micro beams which are separated so that when they strike the skin surface

 

small areas of the skin between the beams are not hit by the laser and left intact. These small areas or bridges of untreated skin promote a rapid recovery and healing with less risk of complications.

This picture shows that fractional lasers remove only small areas of skin leaving bridges of normal skin in between.

 
           
   

Fractional ablative laser resurfacing

Two types of lasers are mainly used for fractional resurfacing, erbium and CO2. Recently a new wavelength YSGG has been introduced [ pearl laser]. We believe CO2 lasers are best in delivering results as other two wavelengths are limited in their capability to ablate, penetrate and heat the skin which CO2 does best. There are now numerous devices capable of performing such treatments. Best known are Lumenis ultrapulse encore and smartxide DOT lasers. Some other lasers are fraxel repair, mixto, ellipse juvia and lutronic eco2.

There is about three to seven days of redness following the procedure and one to three sessions may be required to achieve desired results.
